چکیده

Introduction Lead extraction is a high-risk procedure with potential serious complications, such as vascular avulsion, cardiac tamponade, and death. When a patient is referred for lead extraction, a careful assessment of any alternatives should be undertaken. When lead malfunction is the issue, the risks and benefits of lead extraction versus implantation of additional leads should be considered. Often overlooked is the possibility of reusing old or abandoned leads. We report a case where it was possible to avoid both lead extraction and addition of a new lead by reusing a previously abandoned pace/sense (P/S) lead, with an excellent clinical result.

Percutaneous extraction of a coiled, 20-year-old lead in a patient with cardiac resynchronization therapy.

A 61-year-old patient with a 20-year history of permanent pacemaker implantation and half--a-year cardiac resynchronization therapy using a left ventricular lead placed via surgical approach was admitted for extraction of an old coiled right ventricular lead, which triggered ventricular arrhythmia and created a risk of pulmonary embolism. The lead was extracted via the left femoral vein in two ...

Extraction of abandoned, potentially dangerous lead with uncovered proximal ending: a case report and method description.

In this study, we present the case of the extraction of a non-functioning, abandoned, chronically implanted nine year-old lead with proximal extended frayed ending, displaced spontaneously into the subclavian vein. The seemingly inaccessible lead was extracted from the body using the femoral approach. The lead was looped with a pig-tail catheter, standard guide-wire, and basket Dotter catheter,...

Transvenous extraction of a five year-old ventricular lead inadvertently implanted in the left ventricle.

Inadvertent lead implantation into the left ventricle (LV) is a rare but serious complication of permanent pacing and should be diagnosed as soon as possible. We report a case of a patient with a pacemaker pocket infection with sepsis and two ventricular leads - one old electrode abandoned in the right ventricle and another one unintentionally implanted via patent foramen ovale into the LV. Bot...

Management of renal abscess in a 5-year-old girl: A case report

Renal abscesses are rare in childhood. They have vague manifestations and any delay in proper diagnosis and treatment could lead to morbidity and mortality. We present a 5-year-old girl with multiple renal abscesses after pyelonephritis that lead to total nephrectomy due to multiple drug resistance.  The renal abscess may have subtle symptoms. Early diagnosis and treatment strongly prevents un...

ICD lead abandonment is without risk? A case of “lead on lead crime”

Introduction Lead failure rates of implantable cardioverter-defibrillators (ICDs) vary depending on design and composition, and can be as high as 40% a few years post implant. The Sprint Fidelis lead (Medtronic, Minneapolis, MN) was recalled in 2007 because it is prone to fracture, particularly to its pacesense conductor, which predisposes patients to inappropriate shocks; its yearly failure ra...
